As part of providing a psychological service to you, the psychologists needs to collect and record personal information from you that is relevant to your situation, such as your name, contact information, medical history, and other relevant information as part of providing psychological services to you. This collection of personal information will be a necessary part of the psychological assessment and treatment that is conducted. Please note that you do not have to give all of your personal information however if you choose not to do so then the psychological service may be restricted or may not be able to be provided to you.
Your personal information is gathered as part of your assessment and treatment, is kept securely and, in the interests of your privacy, used only by your psychologist, and the authorised personnel of the practice (as necessary) and any authorised service provider in accordance with the practice's policies and procedures. Your personal information is retained to document what happens during sessions and enables the psychologist to provide a relevant and informed psychological service to you. Please contact your psychologist via email (admin@theshirepsych.com.au) to request access and/or seek correction of your personal information.
At any stage you are entitled to access your personal information kept on file, subject to exceptions in the relevant legislation. The psychologist may discuss with your different possible forms of access. An administration fee may apply.
There are often people on a waiting list requiring an appointment. Therefore, if enough notice is provided when rescheduling and/or cancelling your appointment, these people may be able to access services sooner.
If you need to cancel or postpone your appointment, please give The Shire Psychology at least 24 hours notice, otherwise you may be charged the full cost for the session. Please note cancellation fees are not claimable as a rebate from Medicare or private health insurance.
For all first appointments, we require payment to be made prior to attending the appointment. If this appointment is cancelled with more than 24 hours notice, it can be refunded or moved to a future appointment. If less than 24 hours notice is provided, this payment is used as a cancellation fee payment.
The Shire Psychology has a zero-tolerance policy towards aggressive behaviour of any kind, including raising of voices towards staff members, throwing of items, physically hurting another individual and speaking inappropriately. Should this occur, The Shire Psychology has the right to refuse the provision of services.
There may be times where your psychologist may need to refer you on to an alternative psychologist due to a lack of speciality in the field you require. Should this occur, your psychologist will explain this to you and provide you with a list of referrals that may be better suited.
